Registration Packets - including Sport Physicals

All Registration Packets contain: Registration Form, Consent Form, Athletic Requirements and District Training Rules and one of two Physical Forms.

Packet with Physical - GENERALLY, Freshmen, Juniors, or any student new to Skyline High School must have a current physical form on file BEFORE the athlete is allowed to practice with the team. For the 2009-10 school year, a physical must be dated after May 1, 2008. A physical is good for a MAXIMUM OF 2 YEARS from the date of the exam.

Packet with Interim Physical Questionnaire - GENERALLY, Sophomores, Seniors or any student with a physical exam on file less than two years old. Students complete this form (instead of a physical exam) every other year. The interim questionnaire must be turned in BEFORE a student is allowed to practice with the team.

If you are unable to download the forms you need, you may pick up the packets at the Skyline High School Main office at 1767 Blue Sky Drive. You may e-mail our Athletic Director, Kerry Martin or you may phone him at 525-7798 or 569-5421 to have the packet sent to you via mail, email or fax. If your request is for a physical packet, please indicate the date of the last physical exam that your student athlete received so that we will know what packet to forward to you.
