Requests for transcripts must be received in writing. E-mail and phone requests are not accepted due to the Family Educational Rights and Privacy Act.
Please include full legal name used when attending school, date of birth, name of last District 91 school attended and graduation year (if applicable), and a current phone number in case we have questions. Incomplete information may result in processing delays.
We must have a signature to release the information. Students 18 years or older must sign for release of information. A parent may sign for students under 18. A third party may pick up the transcript with written authorization from the requesting party.
- Regular transcript fee is $5.00 per copy. (Mailed within 5-10 business days after payment is received).
- Rush service fee is $10.00 per copy. (Request is processed the next business day after payment is received.)
You can mail a check payable to School District 91 or send cash with the Transcript Request Form to the address above to process your transcript request.
Online payments are also accepted at e-Funds for Schools. There is a $1.00 convenience fee if you pay with your checking account or $2.45 convenience fee if you pay with a credit card. Once the payment is made the Transcript Request Form must be completed with the online payment information included and received by the district office to process the order.
