Enrollment Instructions

The following information is required when enrolling a student into Taylorview Junior High School:

1 Verification of your address, indicating residence is in Taylorview Jr. High attendance zone. (i.e. Power bill, Rental or Purchase Agreement, Driver’s License)

2 Withdrawal form with grades from previous school. (If enrolling before the school year starts, please bring your final report card from the last school attended or a copy of your transcript.

3 Birth Certificate

4 Immunization Records

5 Legal documentation of guardianship if you are not the biological parent.

6 Please copy off and fill out the following forms. (These are available at the Counseling Office if you would like to pick them up).

A. Student Data Sheet

B. Records Request Form

C. Home Language Survey

D. Medical History Form

E. Tylenol Consent Form (optional)

F. Technology Use Agreement
