Registration Requirements:

• Verification of your address, indicating residence is in the attendance zone. (i.e. Power Bill, Rental or Purchase Agreement.)
  
  
• Withdrawal form with grades, as well as any final grades from last school attended.
  
  
• Birth Certificate
  
  
• Immunization Records - Be aware that the immunization requirements for students entering the 7th grade in the 2011-2012 school year have changed.  See below:

  Children admitted to 7th grade during or after the 2011-2012 school year must meet the following minimum immunization requirements in addition to school entry requirements:

  1 - Tetanus, Diphtheria, Pertussis

  1 - Miningococcal
  
  

• Legal documentation of guardianship if you are not the biological parent.
